The Brain Waste Clearance System
The brain waste clearance system is important for brain homeostasis and neurological health as it is responsible for the efficient clearance of neurotoxins and metabolic waste from the brain and brain tissue, including soluble proteins associated with neurodegenerative disease. 1-3,5-11
The brain waste clearance system is a complex network of fluid channels and mechanisms within brain tissue. It functions as an exchange between fresh cerebrospinal fluid (CSF) flowing into brain tissue, replacing the interstitial fluid that contains accumulated brain waste. 1,2
After the brain waste is flushed out of the brain tissue, it drains into the lymphatic system for elimination from the body.2 In addition to brain waste removal and cleansing, this system has a replenishing function as it helps distribute non-waste compounds such as glucose, amino acids, lipids and neuromodulators to brain tissue, which is important for normal brain function. 1,2,4
The brain waste clearance system function is sleep-dependent, which has led to renewed interest in the health consequences of chronically disrupted sleep and the importance of supporting high-quality sleep to promote both long and short-term brain and cognitive health. 3,4,10,11
A well-functioning brain waste clearance system is important for many aspects of brain health and function, including cognition, memory and mood. While sleep is the key driver of brain waste clearance, several other factors influence the efficiency of waste removal, including arterial pulsation, lymphatic system clearance, nervous system function and neurotransmitter activity. 2,3,4,9-11
CLINICAL EVIDENCE
Sleep
Sleep, especially deep slow-wave sleep, is the primary driver of brain waste clearance.2-4 Circadian rhythms may also influence brain waste clearance.15 Without adequate sleep or with reduced sleep quality, brain metabolic waste and neurotoxins accumulate in the brain, potentially compromising neurological Warning and cognitive function.2,9-12
Lemon Balm
Melissa officinalis (lemon balm) is traditionally used in Western herbal medicine to improve sleep quality and help with mind relaxation. 14-16
Waste Clearance
After clearance from the brain tissue, brain waste ultimately drains into the lymphatic system. 2,10 Poor lymphatic system function and body elimination processes may influence the efficiency of the brain waste clearance system. 10,11,17
Calendula
Calendula officinalis (calendula) is used in traditional Western herbal medicine to support a healthy lymphatic system and to aid natural detoxification processes. 15,18,19
Choline
Choline is a nutrient produced in the liver and is essential for methylation and transporting fats away from the liver. Choline supports healthy liver function and detoxification processes. 20-25
Circulation
Arterial pulsation is a key driver of brain waste clearance and facilitates the movement and flow of CSF into brain tissue. 6,10 Reduced cardiac output may diminish flow as it decreases the pulsatility of the cerebral arteries. 10
Ginkgo
Ginkgo biloba has been shown to improve the circulation of blood throughout the body, including the CNS. 26 Clinical research has shown that Ginkgo increases cerebral and peripheral blood flow, microcirculation and reduces vascular permeability. 26-28
Brain Health, Nervous System Function and Cognition
Brain health, nervous system function and neurotransmitter activity also play a part in the brain waste clearance system. 4 The drop in noradrenaline levels that occurs when we sleep leads to an expansion in the brain’s extracellular space and decreased resistance to CSF flow into brain tissue. 3,4 Removal of metabolic waste from the brain is important for brain function, memory and cognitive health at all stages of life. 9
Magnesium
Magnesium is a cofactor nutrient that helps to maintain nervous system function. 29,30 Magnesium also plays an essential role in nerve transmission. 28 It functions in a protective role against excessive excitation that can lead to neuronal cell death (excitotoxicity). 30
Choline
Choline plays an important role in brain function, neurotransmission and cell membrane signalling. 32-36 Choline is needed to produce acetylcholine, an important neurotransmitter for memory, mood, muscle control, and other brain and nervous system functions. 2,35-39
Choline is also required for the biosynthesis of several phospholipids that are essential components of cell membranes, including those in the central nervous system. Therefore, it may help support the structural integrity of neurons. 32 Choline appears to increase the density and activity of receptors which play a role in normal cognitive processes, including processing speed, attention, working memory and executive function. 38
Ginkgo
Ginkgo has been shown to enhance cognitive performance. 26 Ginkgo’s mechanism of action for improving cognition has been linked to improving cerebral blood flow and its antioxidant properties. 39 Clinical research in healthy adults shows that taking ginkgo leaf extract daily for up to 12 weeks may improve some measures of cognitive function, such as working and long-term memory, attention-based tasks, speed of information processing, executive function, immediate and delayed recall and recognition. 40-50
Centella
Centella asiatica is traditionally used in Ayurvedic medicine to enhance brain health and to improve memory and cognition. 51-53
